Clearly no doctors read package inserts. Found in Moderna's - all bold - "Vaccination providers must report all vaccine administration errors, all serious adverse events... cases of COVID-19 that result in hospitalization or death..." I've seen hundreds of vaccine-injured patients - not a single one was reported to VAERS by other providers.
